Information infrastructure as clinical necessity

Structured information entry, such as forecast discharge dates and digital frailty triggers, goes with process gains in single systems. That it is necessary for clinical outcomes is untested.

Register's basis, before re-grading: Confirmed at the level of association; neither new source tests necessity as distinct from usefulness

Status now

Provisional

Every pattern enters here, never higher. Its first entry names what evidence would qualify, contest or discard it. The publication's own inferences stay here until external evidence or events move them.

Entered the register as established in June 2026, before the promotion tests existed. They were not applied on entry.

How it grades

Graded October 2026 · Very low confidence · two independent passes agreed

The register recorded this pattern as established. Graded against the written rules, it is provisional. The register moves at its next monthly review.

The claim the evidence supports
Structured information entry, such as forecast discharge dates and digital frailty triggers, goes with process gains in single systems. That it is necessary for clinical outcomes is untested.
Starting point
  • D01 silent makes no claim on information infrastructure
  • D03 qualifies documenting a preference did not change where people died
The evidence
2 test or bear on it 0 cited but do not test it 2 not yet read at source 4 endorsements, not counted Domains earned: D01 of the 5 it spans
What would prove it wrong
A comparable system delivering equivalent outcomes without the infrastructure.
What the re-grade found
The sources show association within single systems. None tests necessity, and better-organised hospitals may simply do both.

A grade measures how certain the evidence we have assessed makes the claim as worded: its own sources and the settled evidence for its domains. Low means unproven, not implausible. What a grade means →

The longitudinal record

  1. Established

    Entered the register

SPR legislation, NHS operational planning, NHS England Board Item 8 aligned. PAC found NHS England digital access priorities displaced frailty work. GP collective action on voluntary data-sharing unresolved.

Recorded before the register required linked evidence: the sources are as named above, and the rest are not individually linked.

  2. Established

    Reconfirmed

Confirmed at the level of association. Earlier entry of a medically-ready-for-discharge date was associated with fewer unnecessary bed days (112,930 visits, observational), and a digital assessment trigger reached 71% assessment within four days. Neither source tests necessity as distinct from usefulness. Disconfirmation condition: a comparable system shown to deliver equivalent outcomes without the infrastructure.
