Reablement-based home rehabilitation shows weak evidence of benefit, meta-analysis finds

A GRADE-assessed systematic review and meta-analysis of 27 trials finds reablement-based home rehabilitation has low-to-very-low-certainty evidence of improving daily function, while structured exercise-based rehabilitation does show benefit.

A systematic review and meta-analysis of 27 randomised controlled trials (n=4,948), GRADE-assessed, examined the components and effects of home rehabilitation on activities of daily living and physical performance in community-dwelling older people with low physical performance [Meta-analysis]. It found that reablement-based interventions — coaching patients toward independence without a structured exercise component — showed no significant effect on activities of daily living or physical performance, with low-to-very-low-certainty evidence. Exercise-based home rehabilitation, by contrast, did show a significant benefit.

This is a GRADE-assessed meta-analysis of 27 trials, placing it toward the top of the evidence hierarchy for this question and giving its finding on reablement more weight than any individual service evaluation could carry.

Reablement-first, coaching-based discharge-to-assess is the model much of the NHS relies on for post-discharge community rehabilitation. This finding directly challenges the evidence base underpinning that model, and argues for local audit of outcome data wherever reablement-first pathways are the dominant approach — the value-added-time case for reablement-based discharge, as distinct from structured exercise-based rehabilitation, is not yet well supported by this evidence.
