Blood-based biomarkers materially change diagnostic confidence in memory clinic decision-making
A prospective study across three Netherlands academic memory clinics finds that disclosing a plasma biomarker panel at multidisciplinary team meetings increased diagnostic confidence in 46% of cases and revised the suspected diagnosis in 5%, using far less invasive testing than CSF or PET.
A prospective diagnostic study across three academic memory clinics in the Netherlands (n=450) tested what happens when a plasma biomarker panel (pTau181, GFAP and NfL) is disclosed to multidisciplinary teams during diagnostic decision-making Primary Study. Disclosure increased diagnostic confidence in 46% of cases and led to a revised suspected underlying diagnosis in 5%, using blood testing that is far less invasive than cerebrospinal fluid analysis or PET imaging.
This is among the strongest currently available evidence on the practical value of blood-based biomarkers within everyday memory clinic decision-making, rather than on diagnostic accuracy alone. It was conducted in an academic-clinic population, and the pathway tested is not yet a costed, commissioned clinical service anywhere the evidence base covers.
As blood-based biomarker technology matures, this points to a plausible future direction for memory service diagnostic pathways, offering a less invasive alternative to cerebrospinal fluid testing or PET imaging. It is best read as a watch-list item for future service redesign rather than a change practice should make now.
