Education alone underperforms exercise for patellofemoral pain, but professional-led education matches it by medium term
A systematic review and meta-analysis of 20 randomised trials finds education alone underperforms exercise and multimodal physiotherapy on short-term pain and function for patellofemoral pain, though health-professional-delivered education performs comparably by medium-term follow-up.
A systematic review and meta-analysis of 20 randomised controlled trials, 11 of which were pooled quantitatively, compared education-only interventions against exercise and multimodal physiotherapy for patellofemoral pain and patellofemoral osteoarthritis Meta-analysis. However, education delivered specifically by a health professional performed comparably to exercise or physiotherapy by medium-term follow-up. All comparisons in the review were rated low to very low certainty.
This is a systematic review with meta-analysis, giving it a stronger evidence base than any single trial, though the authors’ own certainty ratings are low to very low throughout — the findings should be read as directional rather than definitive, and further high-quality trials could shift the picture.
The practical caution is against relying on education-only community musculoskeletal physiotherapy offers without face-to-face exercise-based input, at least in the short term, while noting that professionally delivered education may close much of that gap over a longer follow-up period.
