Six European countries' timely-discharge policies share principles of clear responsibility and resourced community capacity

A comparative analysis by country experts identifies 17 policies for timely hospital discharge across Denmark, France, Germany, the Netherlands, Norway and Sweden, and names the principles that underpin many of them.

Timely discharge of patients who are clinically ready to leave hospital is a common health-system problem. A comparative study by experts of the Health Systems and Policy Monitor network of the European Observatory on Health Systems and Policies, together with additional country experts, set out to identify policy options for delayed discharge in six European countries (Denmark, France, Germany, the Netherlands, Norway and Sweden) and to summarise what is known about their effectiveness Synthesis (Eriksen et al., PMID 41861535). Experts identified relevant policies and initiatives up to November 2023 and, where evaluations existed, reported their findings; the authors then carried out a qualitative cross-country comparative analysis.

They identified 17 policies or initiatives. Hospital-based initiatives included discharge planning and transitional care. Community and home-care initiatives included municipal emergency beds, intensive home rehabilitation and assisted discharge. Cross-sectoral initiatives ranged from regional and municipal coordination to decision-support systems and financial incentives. The authors name five principles that underpin many of them: clarity of responsibility, effective planning and communication, resourcing of community-based capacity, the possible unintended consequences of financial penalties, and the need for a systemic approach.

This is an expert-network comparative analysis, not a systematic review or a pooled estimate, and it describes policies rather than measuring their effect on patients. It is reported here from the published abstract: the country-by-country evaluations are not set out, so this record does not say which initiative has the strongest evaluation. The policy search ends in November 2023, and none of the six systems is the UK.

For benchmarking, the value is the design lesson rather than a target: responsibility for the patient at the point of transition and community capacity to receive them appear as common conditions in these policies, and the warning on financial penalties is stated as a possible unintended consequence rather than a demonstrated harm. No outcome in the study is expressed as patient time.
